Fortifying Wellness | Integrative Psychiatry Clinic PLLC (“Fortifying Wellness”) has adopted this Privacy Policy (the “Privacy Policy” or “Policy”) to explain to you how your information is protected, collected, and used. Fortifying Wellness may modify this Privacy Policy from time to time. Fortifying Wellness will provide notice of materially significant changes to this Privacy Policy by posting notice on the Fortifying Wellness website located at https://www.fortifyingwellness.com (the “Site”).
This Privacy Policy applies to information collected through the Site and other information collected by Fortifying Wellness that is not governed by Fortifying Wellness’s Notice of Privacy Practices. This Privacy Policy does not govern protected health information (“PHI”) that Fortifying Wellness creates, receives, maintains, or transmits in its capacity as a health care provider subject to the Health Insurance Portability and Accountability Act of 1996 (“HIPAA”). Fortifying Wellness’s use and disclosure of PHI is governed by its Notice of Privacy Practices (“Notice of Privacy Practices”).
If you are a patient or become a patient of Fortifying Wellness, certain information about you may be governed by the Notice of Privacy Practices rather than this Privacy Policy. To the extent there is any conflict between this Privacy Policy and the Notice of Privacy Practices with respect to PHI, the Notice of Privacy Practices controls.
By using the Site, you acknowledge the data practices described in this Privacy Policy.
Unless you are using our secure patient portal or another communication method specifically designated by Fortifying Wellness for patient communications, please do not submit medical records, detailed medical histories, diagnoses, treatment information, Social Security numbers, or other highly sensitive information through the Site.
Information submitted through the Site for the purpose of seeking health care services may, depending on the circumstances and applicable law, be subject to additional privacy protections. Nothing in this Privacy Policy is intended to limit any rights or protections that apply to information under HIPAA or other applicable health privacy laws.
PROTECTING YOUR PRIVACY
Fortifying Wellness uses commercially reasonable administrative, technical, and physical safeguards to protect your Personal Information. Fortifying Wellness will not disclose your Personal Information to third parties for their own marketing purposes without your consent, except as otherwise permitted by applicable law. “Personal Information” means information that identifies, relates to, describes, or could reasonably be linked to an identified or identifiable individual.
Fortifying Wellness or third parties may provide links to third-party websites, which may have different privacy practices. Fortifying Wellness is not responsible for, and does not have any control over, the privacy practices of those third-party websites, and encourages all users to read the privacy policies of each website visited.
The Site is not directed to children under the age of 13, and Fortifying Wellness does not knowingly collect information from children in violation of the Children's Online Privacy Protection Act (COPPA).
Use of the Site does not establish a patient-provider relationship with Fortifying Wellness. Individuals who become patients of Fortifying Wellness will receive additional documents applicable to the patient-provider relationship, including Fortifying Wellness’s Notice of Privacy Practices and applicable patient and payment agreements.
DATA FORTIFYING WELLNESS COLLECTS
On the Site, Fortifying Wellness may collect the following data:
- name.
- email address.
- physical contact information, including physical address and phone number.
- information that you enter into the Site.
- billing and other information you provide if you make payments for services through our Site.
- information that you send through the Site.
- dispute resolution, correspondence through the Site and/or correspondence sent to Fortifying Wellness by any other means (such as mail, email, contact forms online, telephone messages, etc.).

Fortifying Wellness may collect your phone number. By voluntarily providing your phone number, you consent to receive telephone calls and text messages regarding your inquiry, appointments, or services, as permitted by applicable law. Consent to receive text messages is not a condition of purchasing services. Message and data rates may apply.
Fortifying Wellness is not responsible for the privacy or security practices of third-party websites or services that are not under its control.
Fortifying Wellness may collect Personal Information if you provide it in feedback or comments or if you contact Fortifying Wellness directly. Information submitted through public areas of the Site is not treated as confidential. While Fortifying Wellness takes reasonable steps to protect information submitted through private forms or communications, the Site is not intended for the transmission of highly sensitive or confidential information. Users should not submit highly sensitive or confidential information through public portions of the Site.
Fortifying Wellness’s web logs collect standard web log entries for each page served, including your IP address, page URL, and timestamp. Web logs help Fortifying Wellness diagnose problems with its server, to administer the Site, and to otherwise operate and maintain the Site. In addition, Fortifying Wellness may collect other information from your interaction with the Site, content, and advertising, including computer and connection information, statistics on page views, traffic to and from the Site, advertising data, IP address, demographic and navigational data, browser type, operating system, device type, language settings, referring URL, and standard web log information. Fortifying Wellness may obtain this information from a third-party provider. The Site does not currently respond to Do Not Track signals.
DATA FORTIFYING WELLNESS STORES
Fortifying Wellness may retain certain information as necessary to comply with legal obligations, resolve disputes, and enforce agreements, even after a user requests deletion. This includes, but is not limited to, tax records, payment records, communications, information necessary for legal compliance, and backups.
Web logs and other records are retained only as long as reasonably necessary for business or legal purposes. This may include information regarding services requested or purchased and your interactions with the Site.
Fortifying Wellness uses reasonable administrative, technical, and physical safeguards to store the information in a secure operating environment that is not available to the public. CIRCUMSTANCES IN WHICH FORTIFYING WELLNESS MAY RELEASE INFORMATION

Fortifying Wellness may disclose information about its users to law enforcement officers or others, in the good-faith belief that such disclosure is reasonably necessary to: enforce Fortifying Wellness’s Terms of Use; respond to claims that any posting or other content violates the rights of third parties; or protect the rights, property, or personal safety of Fortifying Wellness, its users, or the general public.
Fortifying Wellness may disclose information relating to payments if a dispute arises regarding a payment made to Fortifying Wellness.

To the extent such information constitutes PHI, its collection, use, disclosure, and protection are governed by HIPAA and Fortifying Wellness’s Notice of Privacy Practices. Nothing in this Privacy Policy modifies or limits the Notice of Privacy Practices or any rights you may have under applicable health privacy laws.
CHANGES TO THIS PRIVACY POLICY
Fortifying Wellness may update this Privacy Policy from time to time. The Effective Date at the top of this Policy indicates when it was last revised. Any changes will become effective when the revised Privacy Policy is posted on the Site. This Privacy Policy constitutes the privacy policy governing your use of the Site and supersedes any prior privacy policies relating to the Site.
